Here's my scenario.
RadGrid with skin set to Inox, allowing adds/updates, EditMode="EditForms" and EditFormSettings as below:

<EditFormSettings>

<PopUpSettings ScrollBars="None" />

<EditColumn UniqueName="EditCommandColumn1" ButtonType="PushButton"></EditColumn>

</EditFormSettings>

 I hit the 'Add' button and all controls display correctly (input controls as well as the 'add' and 'cancel' images).

If I change the skin to WebBlue and hit 'Add', I don't get an image for the 'insert' button, only the 'cancel' button.

I've tested several other skins and most of them are acting the same way as WebBlue.  I can get Inox and Black to work as I would expect them to.

What am I missing?

I think I read somewhere that this is a skin issue introduces in the official release of RadGrid for ASP.NET AJAX. Browse the forum to find the correct image (if posted) in order to apply it or request a fix from the Telerik support. I suppose that the glitch will be resolved for the service pack next week.
