Open a window from the server, then have the results returned to the server

I think this is a pretty normal need, but I cannot find an example that completely answers it. 

Ideally I want to open a modal rad window from the server side, request input from the client, and then return that input back to the origional server side page.  Obviously I realize that it will be a new instance of the serverside page, but I do want it to get back to the parent page, not the modal client.

I know this is a normal flow and I keep finding pieces of the solution but cannot find all of the pieces.

Any example would be greatly appreciated.
