[Solved] How to include textboxes with title, descripion etc

Hi

I Haven't been able to find the documentation for how to correctly include textboxes with
info related to the file being uploaded, and how to access this info in code behind.
Sorry if it is in an obvious place - I can't find it.

My wife would say it is 'where you left it', but that can't be right in this case...
