How to build this form with controls?

I was wondering what you thought the best approach would be to build the attached form.  They want the form to use themes so if I switch the theme, the panel header color changes and the buttons at the bottom change as well.

We were thinking maybe grid listview, but weren't really sure.

What do you think?

I see, but being new, if you make a submit button will it make 1 record in a table? For me it's an "Order form" that will display in a RadGrid. However, in my case I want to beak the "order form" up and base the next part of the form be based on the first.

Ex:
Country if US State and so on.

I know it's possible to create a new record in the grid itself, but that leaves the customer with a very complicated form that would likely be abandoned and called in.

An example of a great form was when Google was selling a cell phone. It first presented you with two choices, new contract and add a phone, that way the used would not see the complex form, just what he/she needed to.

Hello Victor,

You may consider using asp Wizard to separate the forms into different logical sections (dependend on each other) or integrate your own custom logic which displays the individual forms on demand based on the previous user entries. The exact implementation is strictly related to your custom schema.
