[Solved] .aspx file for external window

I was wondering if you guys have the website file for this example, I am having a hard time replicating it because many feilds are unavailable. I am not sure if advancedform actually uses another ascx or other things.

there are custom columnfield i want to add to data base which the scheduler does not have, using advancedform should be a lot easier.
