Make report like this

I want to make report like this :

Fields AAAA
ModifiedBy        Value
ecaron                1

Fields BBBB
ModifiedBy        Value
ecaron                A
sparadis             B

My Datasource contain : FieldName, ModifiedBy, Value

How I can make this. I want to create report in designer not dynamically
