Additional Fields

Hello,
I want to use RadUpload like this link
http://demos.telerik.com/ASPNET/Prometheus/Upload/Examples/AdditionalFields/DefaultCS.aspx
but i want only one text box that should appear after the radUpload File Choose in the same row.$0$0It will be the sequence
RadUpload File choose > additional textBox > Remove botton
it should be in one row.
i have tried so much to do that but failed.
how can i do that.
