To achieve you requirement I can suggest you couple of approaches.
- You can add an additional framework element (as TextBlock) in the Items collection of the layer, at the same position as the push pins to show the corresponding label.
- Or you can use framework elements for both - label and push pin. You can see such example shown in the First Look demo of the map control.
