We've recently started replacing standard WPF controls with their Telerik counterparts in our project and we've noticed that Telerik menu items are not visible at design time where the standard WPF menu items are. Is this a bug?

Hi Alex,
The current implementation of the RadMenu control does not support previewing of nested RadMenuItems in the designer.
However, this is already brought up to the attention of the development team. I also logged a public feature request regarding the nested RadMenuItems design time support. You can cast your vote and follow the item to receive updates on status changes on the following link: https://feedback.telerik.com/wpf/1418468-menu-implement-nested-radmenuitems-design-time-support
I hope that this doesn't cause any inconvenience for you and I'm glad you've found a workaround.
