I have got VS setup with the windows the way I like 'em and the menus and toolbars organised to suit the way I work. If I close and reopen VS, naturally enough, everything loads the way I left it.
However, if I close VS having had the ORM addin loaded, the next time VS starts all of my windows, toolbars, etc are back to some default state. Now, I know that the problem isn't being caused by ORM loading when VS starts as I have the addin set to disabled by default and if I close VS, having not had ORM running this session my saved toolbars are remembered.
I don't know if it's just me or if this is occurring to anyone else with a similar setup (WIN7x64 Ultimate, VS2K8 Pro SP1, ORM 2010.2.714.1) but it's been happening here for a while. I have only just come to the conclusion that it's ORM causing the problem, I have, in the last 7 days, completely rebuilt my machine and installed everything from fresh.

'cos of the problems with the server's ast night and today, I've raised a support ticket for this.

We have refactored and improved parts of the menu handling code for the addin and are currently testing this (successfully) with all versions of Visual Studio. If all goes well this should be available with the next internal build.
