Please see the Appointments Style article in the documentation. There you will find examples of how to set the appointment shape. You'll see a ShapeType, ShapeSize and other properties that you can use.
If this isn't sufficient, you'll need to create a custom renderer and set up the styling on the native side of things using your own custom Xamarin.Android/Xamairn.iOS logic. We have three custom renderer articles on the documentation to get you started (here's the Android article).
