I have already answered your support ticket on the same subject. For convenience I am pasting my reply below:
Currently RadEditor does not support inserting image captions out of the box. This feature is in our ToDo list for one of the next releases of the control.
What we can suggest for now is to implement your own custom dialog that would insert images with the desired code. More information on how to create and implement custom dialogs is available in the control's documentation and our online demo Adding Custom Dialogs.
