File not adding to RadUpload control

When I drag/drop or browse for a file and add it to my RadUpload control it is not appearing in the list of file on the control (not getting added) however no errors are reported and when I examine the event args in the control events the correct file data seems to be there.

What can I do specifically do to determine at which point this seems to be failing?
