EntitiesModel/OpenAccessContext is null after fresh deployment.

Hi,

 

I recently deployed to a production environment and for some reason the context fails to initialize. Everything works fine in the development environment. I've triple checked the connection to the database, the fault is not in the connection. I know this is vague but could you point out some of the most common reasons why the database context might fail to initialize ?

 

Version Q1 2015.
