Hi Steve,
1) Disconnected SQL Server picker
Do you mean SQL server authentication instead of windows credentials?
2) Don't delete and re-create the IIS sites
3) OR delete the IIS Sites, but backup and restore the settings like bindings
As I understand you want to preserve the changes in the configuration files, right?

Yes, I'm pretty sure that's what he means. We have a SQL server that's separate from the domain and uses SQL-based authentication only, and upgrading TeamPulse required us to install both web and database portions on the same server, move the web portion to a web server, and update the connection strings to the SQL server manually. It's very common for installers to ask for authentication info for web- and database servers separately, and we feel this should be the case for the TeamPulse installer also.

Yes. :)

Hello Mikael,
I'm happy to announce that in the upcoming release you can use the SQL authentication in the installer. The new version will be available in the first half of April.
About the preserving changes in the config file it is much more complicated. For that reason we are trying to not put any application configurations in them.
I've created a feedback item on our feedback portal so you can vote for it.
