I switched to the newest cordova version, but my IOS icon does not show up, just the default cordova icon. I re-uploaded the icon image using the tool, but it had no affect.

Hello Stock,

We are aware of this issue which is already identified as a bug on our side. The good news is that, the
developers are already working on it in order to provide a resolution in the very near future. Still, for the time being, the only workaround we can suggest would be to migrate your project to a lower Cordova version (e.g. Cordova4.0.0).

As a side note, concerning the change when submitting applications to the Google Play store, we plan to introduce a support for Cordova 4.1.1 with the next AppBuilder release. You can find more details on the exact planBasically, we will update Cordova 4.0.0 so that you can stick to it instead of migrating to an experimental version (5.0.0).
