I have always answered "no" to the IDFA question when uploading a binary to iTunes Connect, but the recent update (with very few changes) got rejected with the following message:

We have discovered one or more issues with your recent delivery for "Boomerang Rewards". To process your delivery, the following issues must be corrected:
Improper Advertising Identifier [IDFA] Usage. Your app contains the Advertising Identifier [IDFA] API but you have not indicated its usage on the Prepare for Upload page in iTunes Connect.

I use a Facebook plugin and Telerik Analytics plugin.

Does anybody know if the AppBuilder now includes IDFA related code when the app is published, or how else it has gotten there?

Hello Sergei,

Thank you for reporting this here. Unfortunately, it is a known issue in the Facebook plugin on which we are currently working. We believe that a permanent solution could be implemented and once we have it, we will be glad to share it with you.

I hope this works for you.
