I've only had this after I installed JustCode Q2 updates into VS2010.
Now when I hit ALT+TAB to switch between applications there is another VS2010 application that just links to the first Visual Studio.
The picture attached might explain better:
Notice the two Visual Studios open and two other empty icons.

Sorry for the inconvenience. We tracked down the problem with this ghost window right after the official release. This is already fixed in our codebase and the fix will be included in the SP that is to be released in a couple of weeks.
